Neumann University Athletics

Welcome First Year Prospective Student-Athletes and Families,
 
We are very excited to see you this upcoming 2025-2026 sports season. Prior to participation in athletics at Neumann University, you will need to complete pre-participation requirements for medical clearance. We collect these documents and forms with use of the ATS, Athletic Trainer System platform. This platform will allow each user to upload all required documents prior to the start of each preseason. 
 
If you are on a preliminary roster, please complete the steps below to create an account. If you are not on a roster, and you are interested in trying out for a team, you will need to contact the head coach of the sport you have an interest in. If the head coach approves the tryout opportunity, they will provide you the necessary information and add you to the preliminary roster.

If you are a new Student-Athlete to Neumann, you will create an Account in the ATS Athlete portal using the link below that says “Link to ATS- Computer Portal”. From there, you will enter NEW for the Athlete ID and NEW for the password. Once into the system on the General Tab, you will need to change your password and Athlete ID to something of your own choice. You cannot keep “NEW”.  

ALL PRE-PARTICIPATION FORMS ARE DUE IN YOUR ATS ATHLETE PORTAL: AUGUST 1st, 2025
 
Prior to the start of Athletic Participation:

• Complete a Physical Exam with a Medical Doctor using the Neumann University Physical Exam Form.
  • Physical Form
    • Exam must be within six months of your respective sports start date
    • Exam must be signed by a Medical Doctor Only
• Create an account in the ATS Athlete Portal

  • Required on Annual Basis:
    • Complete General Tab (address, phone number, class, etc)
    • View Paperwork Checklist Tab (status of your required clearance paperwork) 
    • Complete Insurance Tab (front and back copy of your health insurance card is required)
    • Complete Contacts Tab (emergency contact names and numbers)
    • Complete Forms Tab (if under the age of 18, some of the following forms will require a parent/guardian signature)
      • Agreement to Participate
      • Cardiac Acknowledgment Form
      • Concussion Statement
      • HIPAA Form
      • ImPACT Baseline Acknowledgment
      • Informed Consent Form
      • Injury and Illness Reporting Form
      • COVID-19 Questionnaire
      • SCAT Baseline Acknowledgment
      • Sickle Cell Trait Testing Form
        • A SICKLE CELL TRAIT TEST RESULT IS REQUIRED FOR PARTICIPATION. 
        • Important message: Due to NCAA policy change as of June 2025, Pennsylvania Newborn Sickle Cell Trait Tests are no longer accepted for clearance to participate (See Sickle Cell Trait Testing Form in your ATS portal for more information).
    • Complete eFiles Tab 
      • Complete and upload Physical Exam Form
      • Complete and upload Health Record & Questionnaire
      • Review the NCAA Concussion Fact Sheet
      • Review the Cardiac Fact Sheet
      • Review the Sickle Cell Trait Info Form

• Health Review/Assessment with Neumann University Sports Medicine Team
• SCAT Baseline Test
• ImPACT Baseline Test
