Returning Student Athletes

I hope you are all doing well. For this upcoming 2026-2027 sports season, the pre-participation paperwork collection and processing will remain within the ATS, Athletic Trainer System platform. This platform will allow each user to upload all required documents as well as electronically complete needed forms prior the start of athletic participation. 
 
If you are a returning Student-Athlete from the previous Neumann sports season, you will already have an ATS Athlete portal Athlete ID and Password. DO NOT create a second acccount. If you forget your login information, please refer to the ATS portal log in page for steps to reset your password. Any other questions, please contact your assigned team Athletic Trainer or the Director of Sports Medicine, Julia Wayock at
wadsworj@neumann.edu.

ALL PRE-PARTICIPATION FORMS ARE DUE IN YOUR ATS ATHLETE PORTAL:
AUGUST 1st, 2026
 
Prior to the start of Athletic Participation:

 
  • Complete a Physical Exam with a Medical Doctor using the Neumann University Physical Exam Form.
    • Physical Form
      • Exam must be within six months of your respective start date
      • Exam must be signed by a Medical Doctor Only
      • If you are a Returning Fall or Winter Sport Student-Athlete and you have attended the on-campus Physical Day in April 2026, you do not need to submit another physical exam or 2 page Health Record & Questionnaire Form. If you missed this Physical Day event with us, you MUST complete a Physical Exam on your own with your Primary Care Physician and submit the Exam and Health Record forms to your ATS Athlete Portal account.
      • If you are a Returning Spring Sport Student-Athlete, there will be a Returning Spring Sport Athlete Physical Exam Day on campus on Sunday September 13th with the Sports Medicine team. Stay tuned for more information on this.

    • Required on Annual Basis:
      • Verify or update your information in the General Tab (update: address, phone number, and class)
      • View Paperwork Checklist Tab (status of your required clearance paperwork) 
      • Verify or update your information in the Insurance Tab (front and back copy of your health insurance card is required)
        • For international students who are studying on an F1 visa at Neumann University, the following plan may be an option for you.
        • International Student Insurance
      • Verify or update your information in the Contacts Tab (emergency contact names and numbers)
      • Complete Forms Tab (if under the age of 18, some of the following forms will require a parent/guardian signature)
        • Agreement to Participate
        • Cardiac Acknowledgment Form
        • Concussion Statement
        • HIPAA Form
        • ImPACT Baseline Acknowledgment
        • Informed Consent Form
        • Injury and Illness Reporting Form
        • SCAT Baseline Acknowledgment
        • Sickle Cell Trait Testing Form
          • Important message: Due to NCAA policy change as of June 2025, Pennsylvania Newborn Sickle Cell Trait Tests are no longer accepted for clearance to participate (Please contact Julia Wayock at Wadsworj@neumann.edu if you have previously submitted a PA Newborn Sickle Cell Trait test for clearance).
      • Complete eFiles Tab 
        • Complete and upload Physical Exam Form
        • Complete and upload Health Record & Questionnaire
        • Review the NCAA Concussion Fact Sheet
        • Review the Cardiac Fact Sheet
        • Review the Sickle Cell Trait Info Form

  • Health Review/Assessment with Neumann University Sports Medicine Team
  • SCAT Baseline Test
  • ImPACT Baseline Test
